Dash Plaque Order Forms

Anyone know what address to mail the dash plaque order forms. I have a 2002 Super Air 210 and want to get my dash plaque updated. I tried the manual address and got a return to sender.

The order form is on the last page of the owners manual. If you dont have one give me a fax number and I will send it to you>

They are in the back of your owner's manual. If you don't have it, get it from the downloads section of this site.

What is the new address to mail the from to? Is it just the new factory address?
I'm assuming the mailing address on the from is outdated.

To order these, the manual originally said attn: Tracy Cumbus-Umpierre. Is there someone else it needs to be addressed to? Also original price was $6 is this still correct?

Marty Mabe does them over on CCF, I don't know how new of a boat he will do, but I had the one for my 93 in about a week.
